To refer a student to the CARE Team, please use the Student of Concern Form link above. We encourage students, faculty and staff to use this form to refer any student who could benefit from additional support resources.
Students – self-referrals are welcome!

Some reasons for concern may include:

Academic performance (ie. struggling with coursework, attendance/lateness issues, etc.)

Other general concerns

When a referral is received, a member of the CARE Team will attempt to follow up directly with the student through a variety of methods including email, phone calls, and direct outreach. We cannot guarantee success with every student, but we will work hard to contact students in a timely manner in order to give them a chance to get back on the path to success.
